Three Ways to Refer

Online Portal

Log in to instED NOW to request a visit for your patient. You'll get real-time updates and visit notes 24 within hours in most cases.

Call Us

Call our Clinical Resource Center at 833-946-7833. We'll take the referral over the phone and handle scheduling.

Have Patients Call

Your patients may be able to call us directly or request through their care manager. We'll verify their eligibility and coordinate with you on their care.

When to Refer

Refer patients who have urgent medical needs but don’t require the emergency department:

  • After-hours acute issues (such as UTIs, respiratory infections, dehydration)
  • Chronic disease flare-ups (such as COPD, CHF, diabetes)
  • Patients with limited mobility who struggle to come to the office
  • If you are unavailable, and your patient needs non-emergent care quickly

Advance Scheduling

You can book visits in advance for:

  • Post-discharge follow-up
  • Patients you’re worried about
  • Chronic disease monitoring

Seamless integration with your Practice

We collaborate with your practice to support patients with certain acute, nonemergency needs when clinically appropriate. We don't replace you — we support you by handling urgent issues when you're unavailable, especially after hours and on weekends.

Real-Time Communication

Providers can view visit status updates live on our platform, and visit notes are available following each visit. If there are visit findings that warrant close follow-up, our team may call you directly.

Comprehensive Notes

Full encounter summaries, test results, and treatment plans are made available to care teams in our portal, via fax, or by EMR through direct address.

Follow-Up Coordination

If we flag a patient as needing a follow-up visit, we will recommend follow-up timing and help schedule appointments with your office as needed.

Frequently Asked Questions

How quickly do you see patients?

Most visits happen within 3-6 hours. Scheduled visits can be booked in advance.

What's your scope of practice?

We evaluate and treat episodic, acute, non-emergency issues that may be appropriate for in-home care. We do not provide ambulance transport and we do not serve as the patient's ongoing primary care.

When do you send patients to the ED?

Patients are sometimes escalated to seek emergency care at the time of triage if they do not meet the clinical criteria for an instED visit based on the triage protocols and require a higher level of care. In addition, patients are sometimes escalated to seek emergency care while instED is in the home based on their clinical evaluation and assessment. In all cases, we arrange safe transfer and coordinate with the receiving facility.

How do I get visit notes?

Notes are available in the instED NOW portal When the visit is complete. We can also fax them to your office or send through EMR direct address.

Can I request visits for patients proactively?

Yes! You can schedule visits in advance for patients you're concerned about

Do you bill me or my practice?

No. We bill the patient's insurance directly. There's no cost to your practice.

What if my patient doesn't have insurance coverage?

We offer self-pay options at transparent flat rates.
